All student athlete MUST complete the following items before being allowed to participate in any team training, including lifting:

Complete forms
Upload sickle cell test results - Sickle cell solubility test only - no other tests will be accepted, no exceptions.
Upload insurance
PHQ-9
Concussion symptom baseline
